Request this form if you are claiming multiple structures on one property or for any commercial property (apartments, duplexes, condominiums, or any commercial property).

Unreimbursed Replacement Claim Form

Request this form if you paid to replace damaged JHBP Roof Shakes before the final approval of the Settlement and without knowledge of the Settlement. (You may be entitled to make a claim for Unreimbursed Replacement if: (1) the replaced roof shakes were JHBP Roof Shakes; (2) you personally own or owned the property on which the JHBP Roof Shakes were installed; (3) the JHBP Roof Shakes were damaged such that they required replacement; and (4) the JHBP Roof Shakes were replaced on the entire roof).
